These are 3 options for Clubs to track their membership. Simply chose 1, 2, or 3 and copy paste it into the Constitution Template. You may choose to use one that is not listed here if you wish, although you MUST have a way to track membership for your Club. This is of the utmost importance for when your Club needs to take a vote.

This is the form that Clubs need to turn in to request approval of an event for service hours. If you turn this in 2 weeks prior to your event, and you get at least 1 Club to turn in a sign in sheet or letterhead stating they actually served there, your Club will be alloted 3 Service Hours. Otherwise, the only thing this form could do is put your event on the Service Projects List. This is a list of events that Clubs can look at ahead of time as an opportunity to serve. It is also the only source the Committee looks at during the appeals process, when Clubs did not complete their hours the prior semester. To know if your event is likely to pass, or to find out more about the appeals process, please see our Service Hour Policies.
